School and Work Physicals

Stickney Public Health District physicians will complete work and school physical forms.  There are two types of school physicals – High School Sport and Child Health Exam.  There is no charge for physical exam.  However, there are charges that may be associated with the physical, such as:  necessary vaccines, lead screening or TB testing.   (Cash and check accepted – Please no credit/debit cards.)

School Physical
The Certificate of Child Health Examination Form, also referred to as the school physical form, is required for entry into daycare, preschool, kindergarten, 6th grade, 9th grade,  college and for transfer students.  Stickney Public Health District Clinics begin completion of these school forms in March.  Beat the rush and don’t wait until the last minute before it is due.
WHAT YOU NEED TO KNOW BEFORE A PHYSICAL EXAM
Please Have Ready for the Clerical Staff:

  1. The Parent’s/Guardian’s current Stickney Township I.D. Card. (link to eligiblilty)

  2. Your picture I.D. – Driver’s License or State I.D. accepted.
  3. Medicaid paperwork/Insurance Information.
  4. Immunization Record of person to have physical exam (unable to do school physical without this information).
  5. Physical Form (if not available, we can supply a form in most cases). Do not complete the immunization section of the physical form.
  6. Please be familiar with and ready to answer questions concerning medical history of the person to have the physical.  You must know current medications, allergies, illnesses, etc.
  7. There is no charge for physical exam.  However, there are charges that may be associated with the physical, such as:  necessary vaccines, lead screening or TB testing.   (Cash and check accepted – Please  no credit/debit cards.)

      Please be aware that a physical is time consuming.  We appreciate your patience due to the amount of paperwork that needs to be completed; immunizations that may need to be given and/or lead testing that may need to be done.  We try to provide the best service in the most efficient time possible.  As a reminder, we fast track and triage clients, so all clients may not be taken in the arrival order. Thank you for your cooperation.
Sport Physicals
Most schools require physicals for sport’s participation.  Schools should provide you with the form that you will need completed.  Stickney Public Health District may have a form available if the school has provided us with a copy.  High schools will usually use the form provided through the Illinois High School Association.  Here is a link to print the form.                       

Work Physicals
Often, new employment will require a pre-employment physical to be completed.  Stickney Public Health District does not have access to company physical forms.  Please bring the form the company is using so the physicians understands what is being required.  Many companies have different requirements depending on the position and duties assigned to the employee.

Stickney Public Health District physicians do not complete “Fit for Duty” forms, Secretary of State Driver’s License Reinstatement Forms, Workman’s Compensation Forms, Insurance Froms, or Disability Forms.
